NRNP/PRAC 6635 Comprehensive Psychiatric Evaluation TemplateSubjective:CC (chief complaint): "Not able to pay attention in school, not very well"HPI: Sarah, a school-aged female, presents to the office due to having a difficult timefocusing and completing tasks at school and at home. During the interview, Sarahappears to be distracted and looks away while being asked questions. The clinician hasto repeat questions multiple times during the interview to get Sarah to answer. Sarahhas a short attention span, easily forgetting the answers, questions, and the discussedtopics. She has poor eye contact throughout the interview. Per the mom, the patient hasa difficult time concentrating in school and forgetting about the assignments almosteveiy day. The patient is not able to sit still on a chair for more than five minutes. Thepatient has a difficult time understanding the material in the textbook as well asremembering its content. The patient consistently loses items in school and at homeand is unable to organize her items. The patient admits to occasional frustration fromschool because of not understanding what the teacher says. The patient also strugglesat home as she is not able to focus on completing assigned tasks. She used to haveimpulsive behavior that is dangerous and was not aware of environmental dangers,such as running to the street, jumping into the pool, climbing the wall, and sticking herhands over the bars in the zoo. The patient denies symptoms of sadness, depression,or thoughts of wanting to hurt themself.
