Aliving willprovide instructions to medical staff regarding end-of-life careand may include provisions regarding organ donation. An advancedirectivemay include a living will, and DNR, and designation of ahealthcare proxy. Ahealthcare proxydesignates a person to makedecisions about healthcare and DNR if the client becomes incapable ofmaking the decisions. Clients should routinely be questioned aboutadvanced directives as they may present at a healthcare organizationwithout the document. Clients who have indicated they desire ado-not-resuscitate(DNR) order should not receive resuscitative treatments forterminal illness or conditions in which meaningful recovery cannotoccur. More specifically, ado-not-intubate(DNI) order may also berequested by clients who, in the case of respiratory failure, do not wantto be intubated.

Typical advance directives include code status, (limitedtreatment measures and Do-Not-Resuscitate [DNR] orders), most oftenstating the client’s wishes regarding end-of-life issues, including feedingdecisions and life support. In order to be legally binding, the document iswitnessed, and clearly states the wishes of the client regarding healthcare–related situations. The directive must be drawn up when thepatient is of sound mind and mentally and emotionally capable of makingthese types of decisions. It is encouraged for every patient to have somesort of advance directive, regardless of age or health. On admission to anacute care facility, every patient should be asked and offered theopportunity to execute advance directives, and a copy placed on thepatient’s record. The nurse has some responsibilities when it comes tohelping patients with advance directives. First, the nurse helps thepatient obtain copies of advance directives, ensuring the forms are filledout correctly. The nurse educates the patient and their family aboutkeeping papers in a safe place and submitting copies to the physician,attorney, and others who may need them. The nurse ensures that copiesof all documents are in the chart, and that they are copied and sent if thepatient is transferred.

Social workersalso respond to complaints of abuse and carry out observations andinvestigations.·Chain of command: Leadership comprises the governing body, chiefexecutive officer, nurse executive, and senior managers, departmentleaders, leaders (both elected and appointed) of staff or departments,and other nurse leaders as well as team members and support staff.The chain of command provides a system by which a client or nursecan appeal for assistance or advice.·Interpreter: Ensures that the client/family are accurately informedand able to communicate needs and wants.

In simpler terms, advocacy isspeaking for those who cannot speak for themselves. Nurses areresponsible for recognizing situations that might compromise patients’rights, and advocating for their patients. Nurse advocates acceptresponsibility to safeguard patient rights, and ensure quality of care forthe patients they care for. The role of nurse advocate involves informingthe patient of their own rights, providing information needed to makeinformed decisions, and supporting patient decisions and autonomy.

Generally, an RN cannot delegate tasksto LPNs/LVNs that require the person to teach, assess, plan, or evaluatebecause these tasks are in the scope of practice of the RN. Members ofthe healthcare team to whom tasks can be delegated include:·Other RNs: May carry out any responsibility in the nursing process towhich they have adequate knowledge and training. May delegateduties to LPNs/LVNs or UAPs.·LPNs/LVNs: May carry out specific tasks and procedures, reportobservations, gather data (vital signs, wound condition), implementinterventions outlined in the care plan (in accordance with scope ofpractice). State laws vary, so some may be able to give medications,administer IV, and delegate to UAPs but others may not.·Assistive personnel (PCTs, CNAs): May carry out routine care, suchas bathing, linen changes, toileting, and vital signs (for stable clients).May not give medications or carry out invasive procedures. State lawsmay vary in relation to allowed practice.

It involves a select panel of care providers who give out theirservices based on a prepayment arrangement between the provider andthe managed care organization. For instance, a hospital may be themanaged care organization, and the physicians who are part of thehospital staff are prepaid for the services they render to patients whoseek care from that institution. HMOs and PPOs are examples of thosetypes of managed care services.Case managementis an individual patient care delivery system specificto the patient, their diagnosis, and their individual needs. The maindifference is that managed care is the function of the health carereimbursement system, while case management is the structure used toprovide individual patient care to affect expected treatments andoutcomes.
